What happens to your private insurance if you leave Germany?

If at some point you decide to leave Germany — for personal or professional reasons — your private health insurance needn't be a problem. There's a clear process to close it correctly and avoid unnecessary costs. Here's how it works.

When and how you can cancel private insurance when leaving Germany

Private health insurance in Germany is tied to your residence and employment situation in the country. When you leave Germany permanently — your employment contract ends and you deregister from the residents' register — the private insurance can be cancelled at that point with no additional penalty period to serve.

The key date is the Abmeldung: the official deregistration from the German residents' register. This usually should roughly coincide with the end of your employment contract, since as long as you have an employment relationship in Germany you remain entitled to the employer contribution and the obligation to be covered continues.

If you deregister before your contract ends, the private insurance must stay active until the last day of your employment relationship, because during that period your company keeps paying its share and you remain contributing to the German system.

If in the future you work in Germany again, you'll be able to take out new private insurance if you meet the income requirements. Although you'll no longer have the terms of your previous contract, the application process is the same as the first time.

How to cancel private insurance correctly when leaving Germany

The process isn't complicated, but it has specific steps that need to be followed in the right order to avoid unnecessary costs.

1. Confirm the exact contract end date

Before starting any procedure, you need to be clear on the date your employment relationship in Germany ends. This date is the reference around which everything else is organised: the deregistration, the insurance cancellation and the notification to the insurer.

2. Complete the Abmeldung

The official deregistration from the German residents' register — the Abmeldung — is the administrative step that formalises your departure from the country. It's done at your city's Bürgeramt, and you should complete it roughly at the same time your employment contract ends, not before.

With the Abmeldung done, inform your private insurer that you're leaving Germany and request cancellation of the policy. You'll need to attach the Abmeldung confirmation as proof. The cancellation takes effect on the employment end date.

Let your employer know that the private insurance is cancelled on the agreed date, so they update their contribution and no unnecessary payments are generated. It's a simple step usually handled alongside the rest of your offboarding.

5. Request confirmation of cancellation

Keep the written cancellation confirmation the insurer will send you. It's a document that can be useful in the future if you return to Germany or if you need to prove your health cover history in another country.

Can you return to public insurance if your salary drops in the future?

Although this page is about leaving Germany, it's also important to know what happens if at some point your income falls below the PKV threshold while you're in Germany. In that case, depending on the cause — reduced hours, a job change, unemployment — you may have to return to the public system.

Switching from private to public insurance in Germany isn't automatic or free to choose. It's regulated and depends on your specific situation. If your income drops involuntarily, there are mechanisms to manage the switch in an orderly way.

Either way, if your situation changes, the most important thing is not to make rushed decisions. There are intermediate options — like adjusting the plan or temporarily reducing cover — before cancelling private insurance altogether.

Premiums already paid aren't refunded. However, part of the reserves built up during your time in private insurance can be transferred if you switch to another PKV before leaving.
